The information collected will only be used by ThinkSmart Health (the owner of LEMR) and your preferred medical distributor for the purpose of supplying you with the LEMR services as defined in our EULA and Privacy Policies. Organisation Information Section
This section provides us with information about your organisation.
Organisation Name
We use this to identify your organisation for identification and accounting purposes.
Organisation's phone number
This will be used to contact your organisation as required.
Organisation's postal address
This will be the address added to future invoices sent via email. We may also post invoices and other information to you at this address as required.
Organisation's street address
This is for confirming your organisations' actual location. We may also need to post you equipment in the future if purchased or required for the purpose of providing support and services.
Organisation's ABN
This is for invoicing and tax purposes.
Contacts Section
This section provides us your organisation’s contact details so we know who to connect with to deliver and maintain LEMR.
Your authorised contact
The authorised contact is the person with the authority to sign up on behalf of the organisation and execute any relevant agreements if required.
Contact for training and support for the use of LEMR
The training and support contact is the person we can contact to organise/provide training and support services for LEMR, including important information and product guides for training and support purposes. We may provide this person with information on updates, outages and other relevant information related to LEMR. This would usually be a nurse or manager within your organisation.
Your practice’s IT support provider
Please provide your preferred IT support contact and/or company details so we know who to contact for administrative access and other technical issues at your organisation that we aren't responsible for though have identified. We will only contact your IT support service provider if required to provide the service and you will generally be notified prior to this occurring.
Your contact for accounts and invoicing
Please provide the contact name and/or company name of your book keeper or accounts person so we know who to contact for invoicing and accounts related purposes.
Installation Section
This section provides us with information that will enable us to install and support your integration solution.
The Electronic Medical Records (EMR) system you use
This could be MedicalDirector Clinical, Best Practice, Zedmed or another EMR system that you use in the practice. We are not yet integrated with all EMR systems out there though we aim to cover as many as we can. For those EMR's we don't yet support, we will try to make direct integration possible subject to many external factors outside of our control, though we can at least provide a simple to use service via the LEMR interface and support services as part of the LEMR service outside of your EMR. We are working on the integration into other EMR systems, these will be posted on our website once they are available.
Devices we would be integrating for you
This is a type of medical devices (such as ECG and Spirometry devices) that you would like to integrate into your EMR using LEMR and their serial numbers so we can ensure that it’s a supported device. We are continually working on the integration of additional devices, these will be posted on our website once they are available.
The organisation you acquired your medical device(s) from
This information will help us to identify where the purchase was made and your preferred rep, where applicable, to improve your service quality. If you’ve used multiple suppliers and/or reps, please provide information on all the suppliers and reps used.
How many computers you would like to have the LEMR client and device software installed on
The installation of the LEMR Client software on 1 to 2 computers is included in the base plan price. This is the computer where the medical devices (such as ECG and Spiro) will be connected.
